Truck driver in I-55 crash that killed 4 did not stop

Police said the November crash happened when the driver of a semi-truck failed to slow as he approached the vehicle in front of him.

MADISON COUNTY, Ill. – The 5 On Your Side I-Team is digging into the details of the crash on Interstate 55 that killed four young women and injured several others.

Police said it happened when a semi-truck failed to slow as he approached the vehicle in front of him.

A 53-year-old man named Mohamed Yussuf Jama was at the wheel. Police said he's from Colorado.

Illinois State Police said there is no indication that the driver attempted to stop before hitting the vehicles in front of him.

Investigators also said they're looking at all possibilities of how this could've happened.

However, November's fatal-crash wasn't the first for the owner of the semi: Ohio-based Forward Air Solution, a national corporation that ships goods all over the country.

Forward Air Solutions has roughly 330 trucks, and close to 700 drivers.

But the Department of Transportation fined them a total of $40,000 in 2014 for various infractions.

First, for failure to test a driver for alcohol after being in an accident, which is a federal rule.

Second, they were cited for not having the documents needed to back up the driving logs of their truckers, showing how far and how soon they reached their destinations.

Why is that important?
Because experts say many semi-accidents are caused because the drivers were exhausted.
In the past two years, Forward Air vehicles have been involved in 38 accidents, 13 which caused injuries, and one fatality.

Forward Air Solutions has not responded to multiple calls for a comment. The I-Team is working to find out if Jama is still employed with the company.
